Initial Assessment
Meet with client and support system to discuss needs and develop a care plan.
Advocacy
Attend appointments with client to serve as an advocate and liaise accurate unbiased information to family.
Weekly Visits
Conduct routine wellness visits to ensure safety and wellness of client during their day-to-day lives.
Discharge Planning
Help bring client home from the hospital and ensure immediate needs will be met and plan for ongoing needs.
Care Team Liaison
Act as the point of contact to manage communication and care needs among the larger clinical team and family.
Medication Management
Set up an ongoing medication system to meet client’s needs.
Who am I?
Hi, I’m Amy and I have been an RN for more than 10 years. I attended the University of Wisconsin undergraduate and received my nursing degree from NYU. I have experience working in hospital settings and home care in NYC and Westchester County. I have solid clinical experience, discharge planning experience and overarching care management experience. I am also a mother, a wife, and a daughter.
I have also been where you are now, feeling overwhelmed and confused about what to do next in the face of a medical crisis. I started Lavender Care Management because I have seen first hand how overwhelming and difficult it can be for a patient to think about logistics when they have a lot of emotional turmoil. I have also seen how hard it is for family or friends to take on care even if they really want to when they have their own busy lives and already taking care of other dependents. I can do the heavy lifting for families so you don’t have to during these times of grief, stress and change. I will be a calming, thoughtful, organized partner to you and your loved ones while we work through how to get you settled and living to your best potential. I look forward to meeting you!
