What is an RN care manager?

As an RN care manager, I will be your eyes and ears when everything feels overwhelming or you or a loved one are not able to provide care. My expertise is in guiding patients and their families through health crises and transitions. I serve as a bridge between my clients and the often-complicated healthcare system, helping them navigate through different scenarios and setting them up for success. Whether it is an unexpected diagnosis or a change in ongoing care, we will work together to make sure you have the right support systems in place, so you can focus on healing and enjoying life. 

We will address living arrangements, medications, medical equipment, and other supportive measures you and your family might need in the short and long term. I will be there when things feel overwhelming or when a loved one is not available.  I will pay special attention to the directives from your doctor, so nothing medically significant gets missed and your treatment can proceed under the best possible terms. I will also be conscientious to your social needs and how to best get you living the way you want to live.  

Why do I need a care manager?

  • Helping the elderly age in place.

  • Conducting routine wellness visits when loved ones are not available to ensure overall health in good condition, ensuring medication compliance.

  • Attend doctors appointments with clients when loved ones can’t and patient needs help explaining what is going on and retaining the information from MD.

  • Organizing your life when an unexpected diagnosis occurs ranging from obtaining medication, medical equipment, arranging home care services, finding and hiring aides or other helpers when you can’t fill that role be it housekeeping, nanny, or caretaker to others.
